The deadline for the
submission of the Education Assistance Fund Application Form for Schoolyear
2012-2013 is April 30, 2012.
Filled-up form (available under
Downloadable Forms tab), together with photocopy of Report
Card/s, latest school id and photocopy of the student's school id, latest income
tax return and/or latest three (3) months of Meralco bills must be submitted by
interested party to Melchor Te at CKSC during office hours for evaluation of
application. Status of application being processed may be inquired by sending a
text message to 09189332537 or 09176657474.
Checks will be issued to qualified
applicants twice within a schoolyear. The first check is issued during the month
of May and the second check issued on the month of October.
